How-to: Create Custom Questions to Collect Donor Information

Collect more information from your donors by creating custom questions.

Custom questions allow you to collect unique information from each of your donors. You can add custom questions to your organization’s Donation Pages, Checkout, Event tickets, and Peer-to-Peer registration forms. 

Custom question examples include, but aren’t limited to: 

  • Select your shirt size.
  • Provide your age/select an age group.
  • How did you hear about us?

Add to Donation Page 

  1. Go to app.memberhub.gives and log in. 
  1.  Select Donation Pages in the left menu. 
  1. Click on the Donation Page title 
  1. Select Edit Donation Page.  
  1. Select Options in the timeline at the top. 
  1. Scroll down to Add custom fields to collect donor information, and click on CREATE A CUSTOM FIELD:

  1. Enter the question or custom field title and select a response type:  custom_info_2.pngSome response types, like multiple choice, also let you add preset answer options. For example, you can ask donors for their shirt size and have them select from a list of sizes. 
  2. (Optional) Select the checkbox next to Require donors to complete this field to make this custom question required. When donors go to check out, they’ll be required to respond before they can complete their transaction. 
  3. Select Save. 

Add to Experience Checkout 

  1. Go to app.memberhub.gives and log in. 
  1. Select My Experiences in the left menu. 
  1. If you have chosen a list view, click on the Experience title. If you have chosen a grid view, click on the Experience image. 
  1. Select Edit Experience.  
  1. Select Options in the timeline at the top. 
  1. Click on the plus sign next to Checkout Options.  
  2. Under Add custom fields to collect donor information, click on CREATE A CUSTOM FIELD:

  1. Enter the question or custom field title and select a response type. Some response types, like multiple choice, also let you add preset answer options:

  1. (Optional) Select the checkbox next to Require donors to complete this field to make this custom question required. When donors go to check out, they’ll be required to respond before they can complete their transaction
  2. Select Save.    

    Add to Event Tickets

    1. Go to app.memberhub.gives and log in.
    2. Select My Experiences in the left menu.
    3. If you have chosen a list view, click on the Experience title. If you have chosen a grid view, click on the Experience image.
    4. Select Edit Experience.
    5. Select Ticketing in the timeline at the top.
    6. Scroll down to Choose how to collect attendee information, and select how you want to collect information from attendees: custom_info_5.png

    7. If you are collecting the same information for all attendees, scroll down to Choose the information you want to collect from all attendees and select Add custom field: custom_info_6.png

      If you are collecting different information for each attendee, select Add custom field when you create each new ticket type: custom_info_7.png
    8. Enter the question or custom field title and select a response type. Some response types also let you add preset answer options: custom_info_9.png
    9. Optional) Select the checkbox next to Require donors to complete this field to make this custom question required. When donors go to check out, they’ll be required to respond before they can complete their transaction. 
    10.  Select Save. 

      Add to Peer-to-Peer Registration 

      1. Go to app.memberhub.gives and log in. 
      1. Select My Experiences in the left menu. 
      1. If you have chosen a list view, click on the Experience title. If you have chosen a grid view, click on the Experience image. 
      1. Select Edit Experience.  
      1. Select Peer-to-Peer in the timeline at the top. 
      1. To collect the same information from all registrants/teams, scroll down to Choose the information to collect from participants/teams during the registration process and select Add custom field: custom_info_10.png

        If you are collecting different information for different registrants/teams, select Add custom field when you create each new registration type:  

      2. Enter the question or custom field title and select a response type. Some response types also let you add preset answer options: custom_info_11.png
      3. (Optional) Select the checkbox next to Require donors to complete this field to make this custom question required. When donors go to check out, they’ll be required to respond before they can complete their transaction. 
        1. SelectSave. 

      4. Good to Know 

        • We use the terms “donors”, “participants”, and “teams” in this article, but this terminology may be different depending on your Experience settings (e.g., “participants” could be “runners”; “teams” could be “groups”; etc.).
